Churn Creek Healthcare

Churn Creek Healthcare (CCHC) is an ambulatory health care center offering high quality healthcare to the American Indian and non-Indian communities for Shasta County and all of Trinity County.

Services include:

• comprehensive medical services
• behavioral health
• nutritional services
• diabetes treatment plans
• urgent care

In addition to clinical services, the CCHC offers a wide range of special events throughout the year which focus on health promotion and disease prevention strategies.

To be eligible to attend the clinic, you must be assigned to Churn Creek Healthcare by Partnership Health. All patients must have a valid Medi-Cal card. If you would like to be assigned to Churn Creek Healthcare, please call Partnership HealthPlan of California at 1-800-863-4115.

The family practice center is open from Monday to Friday at 7:00 AM to 6:00 PM; they are closed daily from 12:30 PM to 1:30 PM for lunch.

The urgent care center is open from Monday to Friday at 7:00 AM to 6:00 PM and Saturday and Sunday at 9:00 AM to 6:00 PM.
